Abstract
Résumé: La détermination de l'arsenic dans les cheveux est réalisée depuis plus de 150 ans. Il y a 15 ans, BAUMGARTNER publie la première étude sur la détection de la morphine par RIA dans les cheveux d'héroïnomanes.Les auteurs montrent, à la lumière d'exemples, l'intérêt de cette analyse en pratique médicale hospitalière.Ainsi en surveillance thérapeutique dans le cadre du suivi anti-épileptique, la mesure du phénobarbital dans les cheveux offre des informations qui peuvent compléter utilement les dosages sanguins.En effet, les cheveux concentrent en moyenne le phénobarbital par un facteur deux par rapport au sang. Il existe une corrélation de groupe entre la concentration en phénobarbital dans les cheveux et la concentration en phénobarbital dans le sérum, ainsi qu'avec l'observation clinique dans des cas typiques. La mesure du phénobarbital dans les cheveux apporte une bonne information sur la longue période, en particulier, quand aucun prélèvement sanguin n'a été réalisé, quand des troubles cliniques sont constatés au cours de la surveillance thérapeutique au long cours.En pratique médicale hospitalière, l'analyse des cheveux s'avère très pertinente dans d'autres situations:—pour la mise en évidence d'un tabagisme passif—pour la recherche d'une exposition médicamenteuse chronique—pour le dépistage des conduites toxicophiles.
Summary: Testing hair has been used since more than one hundred and fifty years for arsenic determination. Fifteen years ago, BAUMGARTNER published the first report on the detection of morphine in the hair of heroin abusers by RIA.The authors illustrate with examples the interest of hair analysis in hospital medical practice.The first interest presented in this paper is epileptic management. Hair analysis during phenobarbital therapy demonstrates hair concentrates phenobarbital during anti-epileptic therapy. A group correlation exists between phenobarbital in hair and phenobarbital in serum, and between phenobarbital in hair and clinic observation in some typical cases. Phenobarbital in hair yields good information over a long period, especially when blood collection has not been made, when clinical disorders are observed on long-term therapeutic observance.In hospital medical practice hair analysis is also useful for the following clinical applications presented in this paper:—hair nicotine as a marker of passive exposure to tobacco—evaluation of pharmaceutical exposure—detection survey of heroin addict.
