<p>The current edition of
<italic>Tropical Infectious Diseases: Principles, Pathogens, and Practice</italic>
is the second edition of this outstanding textbook, which was originally published in 1999 and has become one of the essential resources for this extensive area of medicine. The textbook is in 2 volumes, comprising 3 sections: “Principles and General Considerations,” “Pathogens,” and “Practice: Approach to the Patient in the Tropics.” Volume 1 comprises section I and a large component of section II (covering bacterial and viral infections). Volume 2 covers fungal and parasitic infections. This review covers only volume 1. The 2 volumes are well laid out, with a generous number of illustrations and tables to complement the text. The majority of illustrations are in black and white, probably for reasons of cost, although there are 8 pages of color plates at the start of the volume that display venomous reptiles, arthropods, marine animals, and various clinical conditions that are best suited to color reproduction. A CD-ROM is included with the book that contains all of the text and figures in a portable document file (pdf) format that is accessible via a searchable browser interface in both PC and Mac formats. The editors have successfully achieved a consistently high quality throughout this multiple-author text, which has a list of contributors that reads like a who's who of the tropical medicine world.</p>
<p>Section I, “Principles and General Considerations,” in any other setting would, in its own right, comprise an entire textbook on introductory and current concepts in tropical medicine. It provides a solid introduction to the social and biological issues related to tropical diseases, and opening the section to any page has the reader immediately engrossed in the subject being discussed. The section is composed of 15 chapters that cover diverse introductory topics in tropical medicine, such as social and cultural factors, epidemiology and biostatistics, nutrition, poisons, toxins, vectors, malignancy, and immunology. There are also 2 extremely comprehensive chapters that describe current recommendations for the chemotherapy of infectious agents, which are separated into parasitic diseases (chapter 13) and bacterial, fungal, and viral diseases (chapter 14). The final chapter of section I is a new addition to this volume that describes surveillance for emerging infectious diseases and bioterrorism threats. Many of the chapters in this section make very good use of tables to convey comparative aspects of their subject matter, in the tradition of most successful reference textbooks.</p>
<p>Section II, “Pathogens,” contains chapters on bacterial and mycobacterial (chapters 16–43), spirochetal (chapters 44–46), chlamydial (chapters 47–49), rickettsial and ehrlichial (chapters 50–54), viral (chapters 55–75), and human retroviral infections in the tropics (chapter 76). These chapters are logically laid out in a standard format and are divided into an introduction, a description of the agent, a discussion of related epidemiology, a description of the disease, and discussions of pathogenesis and immunity, diagnosis, treatment and prognosis, and prevention and control. Many of the chapters contain illustrations of transmission cycles that use a standardized format (i.e., entry of agents to humans at the 9 o'clock position and exit at the 3 o'clock position), making the text-based explanations more accessible to the reader. Many chapters in this section deal with infections that exist in both tropical and nontropical areas; however, they have been written and structured such that they are relevant to readers in both areas. It is noteworthy that rickettsial diseases are often lumped into a single chapter in medical textbooks, but in this volume, rickettsial and ehrlichial infections are divided into 5 comprehensive chapters (spotted fevers, typhus, scrub typhus, anaplasma and/or ehrlichia, and Q fever) correctly signifying (in my view) their importance in tropical medicine. The viral subsection covers the major tropical viral illnesses and, for this edition, contains a new chapter on Nipah and Hendra virus infections. Given the interest in possible pandemic influenza and severe acute respiratory syndrome (SARS), the chapter on viral infections of the respiratory tract is topical and provides a comprehensive overview of acute respiratory infections in the tropics, as well as covering the specifics of infection due to influenza virus, respiratory syncytial virus, human parainfluenza virus, rhinovirus, adenoviruses, coronaviruses (both SARS-related viruses and non-SARS-related viruses), and metapneumoviruses. This chapter is complimented by a table that compares each of the viruses and associated clinical syndromes, diagnoses, therapies, and vaccines. Comprehensive chapters detailing numerous arthropod-borne virus infections give this volume true relevance in the tropics. Volume 2 completes the pathogens section, covering fungal and parasitic infections.</p>
<p>This book, with its excellent multidisciplinary and interdisciplinary style, will appeal to students at both the undergraduate and postgraduate levels and to clinicians and/or researchers at any stage of their career, and it is certain to become a “must have” on the bookshelf.</p>
