Language="eng"><AbstractText>Since sexuality is rarely discussed in public, it may be difficult to change policies and community norms on the subject. Participative research or community-level debate can help people recognize the existence of the subject and related concerns. However, even so, such a process can span years and requires the support of key decision-makers. Debate over sexual issues is often initiated by health or education program staff. Next, communities often decide to choose several people to be trained as community counselors who can also function as intermediaries between the community and healthcare services. One example of such a process may be seen in Agomanya, a small town in Ghana, where young women used to be recruited to work as prostitutes in a neighboring country. Their families accepted this arrangement in anticipation of the earnings sent home by their working daughters. But these young women were discriminated against and rejected once they returned home infected with HIV and related diseases. Facing this situation, health workers provoked discussion within the community on how to resolve the problem. Realizing through debate the need for the entire community to cooperate on the problem, community leaders decided to forbid the recruitment of prostitutes in the town and explored other ways for community members to increase their incomes.
