Unfortunately he does not really discuss the major changes in the philosophy and structure of health care services in the 1980s, although Hay would presumably simply extend his chronological account, not reshape argument. Hay is concerned that a new environment of social stratification is being created in New Zealand, one in which personal development and well-being are related increasingly to the means of consumption, not just to the means of production (p.6). At one end of the major social division he sees emerging are those able to satisfy their consumption needs through personal ownership (for example, medical insurance), while at the other end of the division are those who rely on the State for their consumption. The nature of New Zealand capitalist society is seen as being behind these developments. Such an argument is hardly new. The inequalities in health have long been established. Socio-economic factors like income, employment (or lack of it), environment, education, housing, transport, and life-style have been shown to have a direct impact on health, individually and collectively. Those to the bottom of the socio-economic pile generally have poorer health, poorer access to health services, and do not live as long as those to the top. But Hay is more concerned with tracing the interactions over time of the different elements in New Zealand, the State, the insurance societies, the public 85 and private hospitals and other entities in the health care arena. Although motivated by day-to-day activities the many different people involved have unwittingly brought about the movement of capital from one circuit (primary, secondary, tertiary) to another. Aspects of the caring health relationship were drawn into the commodity arena and a variety of aspects of health care became socialized and privatised. (pp. 11-12) As this has taken place the relationship between the provider and the recipient of health care has altered, to the disadvantage of those to the bottom of the socio-economic pile. There is much information and data (based on secondary sources), comment and observation which makes for interesting reading: the impact of the influenza pandemic during 1918; the Labour Party with its strongly socialist roots; the emergence of the lodges, friendly societies and insurance schemes; the importance of legislation on the provision of health care, especially the Public Health Act 1920, the Social Security Act 1938, the Hospital Act 1957, and the Accident Compensation Commission Act 1972; the interplay between the cost to Government and the cost to the consumer; the role of the unions; the way the cash-for- service exchange has been adapted and preserved; and the impact of the various pesonalities and organisations on the debates and policies. Hay is really much more concerned with issues of public policy and administration, and with pressure groups. His analysis would have been the sharper had the insights from these fields been applied and discussed. Structural insights would have lost none of their significance and the overall argument would have been greatly strengthened. The argument used here is a conservative one and, it seems, looks back to the time when women provided much more of the health care. "'Free-for-service' doctor performed tasks that were once the domain of women at home". (p.4) Maybe for some people there are real merits in a more capitalist approach. The health field is an immensely complex one and rather more than a structuralist approach is needed to make sense of it. The substantive sections of Hay's book are well worth reading. The theoretical framework and development is limited.</meta-value>
