Abstract
A 59-year-old woman presented with asymmetric parkinsonism that was not-responsive to Levodopa therapy. She developed early falls, postural instability, and moderately severe autonomic dysfunction. Eye movement abnormalities were reported 2 years after her first symptoms, and she eventually developed anterocollis. Experts discuss the syndromal diagnosis and predict the underlying pathology. The pathological diagnosis is given and clinical learning points are considered.
