Abstract

Resume: Evénement fréquent et redouté des situations granulopéniques de toute origine, la fièvre est le plus souvent liée à une infection bactérienne surtout, fongique parfois. Sa survenue impose d'urgence une antibiothérapie présomptive, couvrant à priori les entérobactéries, le pyocyanique, les staphylocoques, les streptocoques, éventuellement guidée par la suveillance microbiologique préalable du patient et des infections hospitalières environnantes. Une association synergique, bactéricide, à large spectre, est indiquée. En attendant la mise à l'épreuve de molécules nouvelles, on utilise le plus souvent une betalactamine associée à un aminoside. Au cas où l'infection est secondairement documentée, l'antibiothérapie initiale est ajustée aux données bactériocliniques. La persistance au 4ème jour d'une fièvre isolée, ou associée à une pneumopathie, peut faire ajouter l'amphotéricine B dans l'hypothèse d'une mycose, difficile à prouver. Les transfusions de granulocytes sont l'objet de controverses.
Summary: Fever happens quite often among patients with granulopenia of several origins ; the fever is mostly related to bacterial infections, sometimes to fungal infections. Its outcome needs in emergency the use of an empirical antibiotherapy aimed either on Gram-negative bacilli, enterobacteries and Pseudomonas, or on staphylococci and streptococci. Patients microbiological follow up and surrounding nosocomial infections survey may help in the choice of the antibiotherapy. The antibiotics need to have wide range activity, to be bactericidal and synergistic in order to be associated in the treatment of granulopenic febrile patients : expecting well assessed new drugs, the associations mainly employed in first purpose are betalactamines with aminoglycosides. If the infection becomes documented, the initial antibiotherapy must be adapted following clinical and bacteriological data. A fever related to a pneumopathy and a fever of unknown origin persisting later than 4 days suggest a possible fungal infection and amphotericin B may be added. Granulocytes transfusions are a point of controversy.

Sa survenue impose d'urgence une antibiothérapie présomptive, couvrant à priori les entérobactéries, le pyocyanique, les staphylocoques, les streptocoques, éventuellement guidée par la suveillance microbiologique préalable du patient et des infections hospitalières environnantes. Une association synergique, bactéricide, à large spectre, est indiquée. En attendant la mise à l'épreuve de molécules nouvelles, on utilise le plus souvent une betalactamine associée à un aminoside. <p>Summary: Fever happens quite often among patients with granulopenia of several origins ; the fever is mostly related to bacterial infections, sometimes to fungal infections. <ce:simple-para>Evénement fréquent et redouté des situations granulopéniques de toute origine, la fièvre est le plus souvent liée à une infection bactérienne surtout, fongique parfois. Sa survenue impose d'urgence une antibiothérapie présomptive, couvrant à priori les entérobactéries, le pyocyanique, les staphylocoques, les streptocoques, éventuellement guidée par la suveillance microbiologique préalable du patient et des infections hospitalières environnantes. Une association synergique, bactéricide, à large spectre, est indiquée. En attendant la mise à l'épreuve de molécules nouvelles, on utilise le plus souvent une betalactamine associée à un aminoside. <abstract lang="fr">Resume: Evénement fréquent et redouté des situations granulopéniques de toute origine, la fièvre est le plus souvent liée à une infection bactérienne surtout, fongique parfois. Sa survenue impose d'urgence une antibiothérapie présomptive, couvrant à priori les entérobactéries, le pyocyanique, les staphylocoques, les streptocoques, éventuellement guidée par la suveillance microbiologique préalable du patient et des infections hospitalières environnantes. Une association synergique, bactéricide, à large spectre, est indiquée. En attendant la mise à l'épreuve de molécules nouvelles, on utilise le plus souvent une betalactamine associée à un aminoside. Au cas où l'infection est secondairement documentée, l'antibiothérapie initiale est ajustée aux données bactériocliniques. La persistance au 4ème jour d'une fièvre isolée, ou associée à une pneumopathie, peut faire ajouter l'amphotéricine B dans l'hypothèse d'une mycose, difficile à prouver. Les transfusions de granulocytes sont l'objet de controverses.</abstract>
<abstract lang="en">Summary: Fever happens quite often among patients with granulopenia of several origins ; the fever is mostly related to bacterial infections, sometimes to fungal infections. Its outcome needs in emergency the use of an empirical antibiotherapy aimed either on Gram-negative bacilli, enterobacteries and Pseudomonas, or on staphylococci and streptococci. Patients microbiological follow up and surrounding nosocomial infections survey may help in the choice of the antibiotherapy. The antibiotics need to have wide range activity, to be bactericidal and synergistic in order to be associated in the treatment of granulopenic febrile patients : expecting well assessed new drugs, the associations mainly employed in first purpose are betalactamines with aminoglycosides. If the infection becomes documented, the initial antibiotherapy must be adapted following clinical and bacteriological data. A fever related to a pneumopathy and a fever of unknown origin persisting later than 4 days suggest a possible fungal infection and amphotericin B may be added. Granulocytes transfusions are a point of controversy.</abstract>
