Abstract
Massive scrotal lymphedema, or elephantiasis, has been described in patients with chronic inflammation of the inguinoscrotal region, filariasis, and following inguinal irradiation and/or lymphadenectomy. Hidradenitis suppurativa, first described by Verneuil in 1854, is a chronic recurrent inflammation of the apocrine sweat glands in the axilla, groin, and perineum. Surgical resection of all involved tissue is the definitive therapy. Acute suppuration associated with hidradenitis suppurativa is treated with antibiotics and surgical drainage. Prophylactic antibiotics and antistaphylococcal vaccines have been used but are not uniformly effective. The association of elephantiasis and hidradenitis suppurativa is not well recognized. We report a case of hidradenitis suppurativa of the perineum and scrotum associated with scrotal elephantiasis, treated with wide local resection and reconstruction.
