<meta-value> from Belgium, India and Israel provide interna- tional interest. The author of the first chapter has lymphoedema and describes the patient's perspective of this incurable, disabling condition. A patient-centred, holistic approach is emphasized throughout the book. Likewise, the importance of the multidis- ciplinary team is highlighted by contributions from members of the medical, nursing and physiotherapy professions. Together these provide a welcome and much needed compilation of current undertstanding of what happens when the lymphatic system fails and how lymphoedema can be managed. Topics dis- cussed include 'how does tissue swelling occur?', 'acute inflammatory episodes', 'drug treatment for lymphoedema' and 'pneumatic compression therapy', all considered in the light of recent research. Each of the 23 chapters in this well-referenced, well-indexed work can be read independently. The text is clear and well illustrated, and this book will be a valuable resource for all health-care profes- sionals involved in lymphoedema services, indeed for all wishing to improve their knowledge of the lymphatic system. Cooke H 2000: When someone dies: a practical guide to holistic care at the end of life. Oxford: Butterworth-Heinemann. 182 pp. £15.99 (PB). ISBN 0 7506 4094 4. This book would be a useful starting point for those new in posts dealing with dying patients and their relatives, covering as it does practical issues faced by professionals on a daily basis. It is clearly aimed at nurses, although it would do no harm for junior doctors to have sight of some of the common sense if somewhat superficial advice on communication with the dying, choices for the dying and control of symptoms. The thumbnail sketches in the last sec- tion on religious traditions and health care I found most helpful. Much of the book is taken up with a most com- prehensive list of contact addresses and telephone numbers. While this is to be commended, it is clear that these will quickly become outdated and unless regularly revised confidence in their accuracy will fade (e.g. Macmillan Cancer Relief has already moved to a new address in London). The produc- tion of a website with this information might be an appropriate next step. Two other factors will limit its appeal. First, there is no effort to tailor advice for staff caring for those dying at home or, indeed, any discussion about how hospital staff might establish the wishes of patients and relatives about preference for place of death. Secondly, there is no acknowledgement of the differences in the legal system in Scotland relating to issues surrounding death (e.g. no coroner in Scotland, different laws on competency, etc.). In summary then, a competent introductory book with a short shelf-life for hospital-based nurses in England and Wales. Symptomatic and supportive care for patients with advanced cancer and AIDS, third edition. Oxford: Oxford University Press. 421 pp. £25.00 (PB). ISBN 0 19 550647 2. The section on AIDS has been expanded in this third edition and covers the treatment of HIV, opportunistic infections, AIDS-related cancers, and relevant aspects of symptom control and palliative care. The rest of the book focuses on cancer. It provides a broad overview of all aspects of palliative care, from pain and symptom management, to psychological, social, cultural and ethical issues. It does not go into great depth, but I think would be excellent for medical students and non-specialists. A list of references for further reading accompanies each section, but individual statements are not referenced, making it difficult to follow through on the evidence base. I like the emphasis on making accurate diagnoses and the need to tailor management to the stage of disease, exploring timeliness and appropriateness of treatment options. Following each symptom sec- tion is a paragraph on management in the terminal phase. There are useful sections on aspects not always covered in a book this size, including practical 82 Book reviews 15PM406 9/1/01 4:06 pm Page 82 guidance on stoma and fistula care, sexual dysfunc- tion, haematological problems and oncology treat- ment side-effects. Working in Australia, Roger Woodruff brings a global perspective to discussions. Symptom manage- ment, however, can be different from that practised in the UK. Largely this is subtle, but, for example, levomepromazine is not mentioned in the section on nausea and vomiting. Overall, this book provides a comprehensive and practical approach to palliative care in patients with cancer and AIDS, promoting a holistic approach to care. Portenoy RK, Bruera E eds 2000: Topics in palli- ative care, Volume 4. Oxford: Oxford University Press. 288 pp. including index. £39.50 (HB). ISBN 0 19 513219 X. This is the fourth volume in a series, which has two aims: to disseminate the findings from the advancing evidence base in palliative care, and to highlight the diversity of issues facing professionals in this field. The book reflects the clinical practice and culture of North America. The first chapter describes survival estimation in palliative care patients. This gives a useful account of the known relevance of symptoms with regard to prognosis. The following chapter on audit was useful and concise, and those on education underlined the importance of involvement in undergraduate education. The last section on symptom control was very useful. The review of intraspinal analgesia showed the accumulating experience of those using the technique frequently. The chapter on delirium illustrated the difficulty interpreting studies in palliative care: lots of small studies, often retrospective, using different criteria. It was refreshing to see the doctrine of 'double effect' discussed in relation to sedation rather than the use of opioid analgesia. The only disappointing section was that on anticoagulation, perhaps because that is my own field of interest. The literature is well reviewed, and the management of venous thromboembolism in cancer patients in general is clearly summarized. Palliative physicians face a number of difficulties in managing these situations, and the chapter on prophylaxis does acknowledge these dilemmas and underlines that there is little evidence to help us in these decisions. This volume would be a useful addition to the library alongside the previous three volumes, and would be an authoritative source of reference.
