Abstract

Abstract: Critical anchorage during orthodontic treatment in the mandible needs both time and effort and patient compliance. In 8 patients, 12 bicortical titanium screws (BIS) were used as anchorage units for orthodontic molar protraction. The criteria for patient selection were: critical anchorage in the lower jaw (i.e. retraction of anterior teeth undesirable) and molar extraction sites. After insertion of the screws in local anesthesia, orthodontic forces were applied immediately. One screw worked loose and had to be removed before the end of treatment. Problems encountered included impingement of the screw head and slight inflammatory reactions of the surrounding mobile mucosa, which necessitated premature removal of two screws. After healing, a new insertion site was chosen. Further treatment was uneventful. Anchorage for orthodontic forces as described offers several advantages. The total treatment time is reduced as the screws can be loaded immediately. The line of action of the orthodontic force coincides with the level of the center of resistance of the molar resulting in a favorable translatory tooth movement. Treatment does not depend on patient cooperation.

<head>Résumé</head>
<p>L’ancrage durant le traitement orthodontique dans la mandibule nécessite du temps, de l’effort et un patient collaborant. Chez huit patients douze vis en titane bicorticales (BIS) ont été utilisées comme ancrages pour la protraction molaire orthodontique. Les critères pour la sélection des patients étaient: ancrage critique dans la mandibule (c.‐à‐d. rétraction des dents antérieures non‐désirables) et des sites molaires d’extraction. Après l’insertion des vis sous anesthésie locale, des forces orthodontiques ont été immédiatement appliquées. Une vis est devenue lâche et a du être enlevée avant la fin du traitement. Les problèmes encourus comprenaient l’empiètement de la tête de la vis et des réactions inflammatoires légères de la muqueuse avoisinante qui ont nécessitées l’enlèvement prématuré de deux vis. Après l’enlèvement, un nouveau site d’insertion a ètè choisi. Le traitement qui a suivi n’a pas présenté de problème. L’ancrage pour des forces orthodontiques comme décrit offre différents avantages: le temps total du traitement est réduit parce que les vis peuvent être chargées immédiatement. La ligne d’action de la force orthodontique coincide avec le niveau du centre de résistance de la molaire résultant en un mouvement dentaire translatéral favorable. Enfin, le traitement ne dépend pas de la coopération du patient.</p>

Bei 8 Patienten wurden zur orthodontischen Protrusion der Molaren 12 bicorticale Titanschrauben (BIS) als Verankungseinheiten eingesetzt. Die Kriterien der Patientenauswahl waren: erschwerte Verankerung im Unterkiefer (z.B. Retroklination der Frontzähne unerwünscht) oder Situationen mit fehlenden Molaren. Nach dem Einsetzen der Schrauben unter Lokalanästhesie wurden sofort orthodontische Kräfte angesetzt. Eine Schraube löste sich und musste vor Behandlungsende entfernt werden. Andere auftretende Probleme waren die Abstossung des Schraubenkopfes und eine damit verbundene leichte Entzündungsreaktion der umgebenden beweglichen Schleimhaut, welche ein vorzeitiges Entfernen von zwei Schrauben nötig machte. Nach Abheilung wurde eine neue Insertionsstelle ausgewählt. Die Weiterbehandlung war dann unauffällig. Die Verankerung für orthopädische Kräfte in der beschriebenen Art und Weise gibt uns verschiedene Vorteile: Die Gesamtbehandlungszeit kann, da die Schrauben sofort belastet werden können, stark verkürzt werden. Die Kraftlinie der orthodontischen Kraft geht durch das Zentrum der Widerstandesebene der Molaren, so dass eine günstige transversale Zahnbewegung zustande kommt. Die Behandlung ist aber auch von der Patientenkooperation unabhängig.</p>

<p>El anclaje crítio durante el tratamiento ortodóntico en la mandíbula necesita tiempo y esfuerzo y conformidad del paciente. Se usaron en ocho pacientes doce tornillos bicorticales de titanio (BIS) comunidades de anclaje para protracción molar ortodóntica. Los criterios para la selección de pacientes fueron: anclaje crónico en la mandíbula (ie retracción de los dientes anteriores indeseada) y lugares de extracción molares. Tras la inserción de los tornillos bajo anestesia local se aplicaron fuerzas ortodónticas inmediatamente. Un tornillo se aflojó y tuvo que ser movido antes del final del tratamiento. Los problemas encontrados incluyeron la intrusión de la cabeza del tornillo y ligeras reacciones inflamatorias de la mucosa móvil circundante, que necesitaron de la retirada prematura de dos tornillos. Tras la cicatrización se eligió un nuevo lugar de inserción. El tratamiento posterior transcurrió sin problemas. El anclaje para fuerzas ortodónticas tal como se describe ofrece diversas ventajas: El tiempo total de tratamiento se reduce al poder cargar los tornillos inmediatamente. La línea de acción de las fuerzas ortodónticas coincide con el nivel del centro de resistencia del molar resultando en un movimiento dental traslatorio favorable. El tratamiento no depende la cooperación del paciente.</p>

<b>Abstract:</b>
Critical anchorage during orthodontic treatment in the mandible needs both time and effort and patient compliance. In 8 patients, 12 bicortical titanium screws (BIS) were used as anchorage units for orthodontic molar protraction. The criteria for patient selection were: critical anchorage in the lower jaw (i.e. retraction of anterior teeth undesirable) and molar extraction sites. After insertion of the screws in local anesthesia, orthodontic forces were applied immediately. One screw worked loose and had to be removed before the end of treatment. Problems encountered included impingement of the screw head and slight inflammatory reactions of the surrounding mobile mucosa, which necessitated premature removal of two screws. After healing, a new insertion site was chosen. Further treatment was uneventful. Anchorage for orthodontic forces as described offers several advantages. The total treatment time is reduced as the screws can be loaded immediately. The line of action of the orthodontic force coincides with the level of the center of resistance of the molar resulting in a favorable translatory tooth movement. Treatment does not depend on patient cooperation.</p>

<p>L’ancrage durant le traitement orthodontique dans la mandibule nécessite du temps, de l’effort et un patient collaborant. Chez huit patients douze vis en titane bicorticales (BIS) ont été utilisées comme ancrages pour la protraction molaire orthodontique. Les critères pour la sélection des patients étaient: ancrage critique dans la mandibule (c.‐à‐d. rétraction des dents antérieures non‐désirables) et des sites molaires d’extraction. Après l’insertion des vis sous anesthésie locale, des forces orthodontiques ont été immédiatement appliquées. Une vis est devenue lâche et a du être enlevée avant la fin du traitement. Les problèmes encourus comprenaient l’empiètement de la tête de la vis et des réactions inflammatoires légères de la muqueuse avoisinante qui ont nécessitées l’enlèvement prématuré de deux vis. Après l’enlèvement, un nouveau site d’insertion a ètè choisi. Le traitement qui a suivi n’a pas présenté de problème. L’ancrage pour des forces orthodontiques comme décrit offre différents avantages: le temps total du traitement est réduit parce que les vis peuvent être chargées immédiatement. La ligne d’action de la force orthodontique coincide avec le niveau du centre de résistance de la molaire résultant en un mouvement dentaire translatéral favorable. Enfin, le traitement ne dépend pas de la coopération du patient.</p>
