Abstract

The activity of health establishments is complex because of the numerous care services, the multiplicity of speakers and trades, competences and care procedures which accompany each pathology. Constraints are coming stricter and stricter, of a medical nature, human, ethical or social nature but also of a financial, regulatory or even political nature. New situations, sometimes unknown or even unexpected, can lead to appearance of risks and damages at the same time to patients (iatrogenic events) and to services concerned, impacting the performance. To face these situations, the establishments try to set up an organization which corresponds more and more to industrial systems organization, placing customer in the center of the concerns. However, the parallel and the total transposition of industrial tools prove to be difficult. Our doctorate work will concern an inevitable process in a care establishment : the medication use system. Through management of the iatrogenic risks, the evaluation and the improvement of its performance will be searched, first in the form of a model (plan) and then through a concrete application in a hospital and university complex (CHU Nice).

            <abstract xml:lang="fr">L'activité des établissements de santé est complexe du fait des nombreux services de soins, de la multiplicité des intervenants comme des métiers, des compétences et des procédures de soins qui accompagnent chaque pathologie. 
Les contraintes sont de plus en plus strictes, d'ordre médical, humain, éthique ou social mais aussi d'ordre financier, réglementaire ou même politique. Les situations nouvelles, parfois méconnues ou même inattendues peuvent entraîner l'apparition de risques et de dommages à la fois au patient (événements iatrogènes) et aux services concernés, impactant la performance. Pour y faire face, les établissements tentent de mettre en place une organisation qui correspond de plus en plus à celle des systèmes industriels plaçant le client au centre des préoccupations. Cependant, le parallèle et la transposition globale d'outils industriels s'avèrent difficile. 
Notre travail de doctorat portera sur un processus incontournable dans un établissement de soins : le circuit du médicament. Au travers du management des risques iatrogènes, l'évaluation et l'amélioration de sa performance seront recherchées, d'abord sous forme de modèle puis en application concrète au niveau d'un centre hospitalier universitaire (CHU Nice).</abstract>
