Abstract
Zusammenfassung: Krankenhäuser der Schwerpunkt- oder Maximalversorgung sind aufgrund der vorhandenen personellen, baulichen, räumlichen, apparativ-technischen und instrumentellen Voraussetzungen mit Sicherheit am besten geeignet, Polytraumatisierte definitiv zu versorgen. Wenn sich Krankenhäuser der Grund- und Regelversorgung an der Definitivversorgung beteiligen, müssen bestimmte Minimalanforderungen erfüllt sein. Entscheidend für die Prognose des Polytraumas ist das initiale Management. Klare Absprachen, wer zu welchem Zeitpunkt die Koordination übernimmt, sind unabdingbar, wobei im ganzen Team Anästhesist und Chirurg am meisten harmonieren müssen. Dabei können kleinere Abteilungen sogar organisatorische Vorteile besitzen, die unter Umständen Nachteile hinsichtlich der personellen oder apparativ-technischen Voraussetzungen ausgleichen.

If basic hospitals should want to take part in the definitive treatment, they would have to meet certain minimum requirements. Initial management is decisive for the prognosis of multiple trauma. It is absolutely necesary to make clear arrangements, who undertakes the coordination and when, and it is important that of the whole team the anaesthesiologist and the surgeon harmonize the most in their cooperation. Wenn sich Krankenhäuser der Grund- und Regelversorgung an der Definitivversorgung beteiligen, müssen bestimmte Minimalanforderungen erfüllt sein. Entscheidend für die Prognose des Polytraumas ist das initiale Management. Klare Absprachen, wer zu welchem Zeitpunkt die Koordination übernimmt, sind unabdingbar, wobei im ganzen Team Anästhesist und Chirurg am meisten harmonieren müssen. Dabei können kleinere Abteilungen sogar organisatorische Vorteile besitzen, die unter Umständen Nachteile hinsichtlich der personellen oder apparativ-technischen Voraussetzungen ausgleichen.</abstract><abstract lang="en">Summary: Certainly Trauma centers are the most suitable institution to give patients with multiple trauma the best definitive treatment, as they meet all the necessary qualifications regarding staff, technical and instrumental equipment as well as adequate accomodation and construction of the buildings. If basic hospitals should want to take part in the definitive treatment, they would have to meet certain minimum requirements. Initial management is decisive for the prognosis of multiple trauma. It is absolutely necesary to make clear arrangements, who undertakes the coordination and when, and it is important that of the whole team the anaesthesiologist and the surgeon harmonize the most in their cooperation.
